ELIGIBILITY FOR SPORTS:

  • Students must be in good academic standing and have received less than five (5) detentions and/or two (2) suspensions during the current quarter to participate.

  • Students who meet any of the following criteria will have their “good” academic status revoked:

  • Average below 65 in two or more subjects (F & F)

  • Average below 65 in one subject (F) and an average between 64 and 70 (D) in one or more additional subjects

  • Average between 64 and 70 (D) two or more subjects

Academic standing will be re-evaluated for team members each quarter.  A student may reestablish good academic standing at  mid-term.

Students must have insurance for athletic activities.

Students must submit emergency contact information.

DEADLINES FOR PARTICIPATION:

Documents "A" (TMS Interscholastic Sports Permission Slip) and "B" ( Connecticut Pre-participation Sports Evaluation) below must be submitted to the Nurse's Office at Tolland Middle School on or before the following dates for the students to participate in the any aspect of the sport, including tryouts. If you have missed the deadline, you may download and submit form C to the Principal's Office for consideration. Forms will only be accepted after the deadline in cases of extreme hardship. Families should plan well ahead of these deadlines and follow through with doctors' offices to allow enough time for paperwork to be submitted before the deadline.

It takes the school Nursing Office time to process and verify these forms, and a large number of forms come in for each sport. In order to execute due diligence in these responsibilities, the TMS Nurse’s Office must have the time to do so carefully. The Connecticut Interscholastic Athletic Conference requires physicals to participate in school sports (Page 98, CIAC 2010-2011 Handbook) and the Connecticut State Medical Society (as of September 2002, Page 5 of SportsMed) recommends a physical be completed a minimum of two weeks before the sport begins. Therefore, in the interest of executing our responsibilities in a responsible manner, and for the safety of the students, we have established deadlines for the submission of completed forms.

ACHIEVEMENT FIRST HARTFORD ACADEMY:  Take Rte. 84 W to Exit 61, I-291 W.  Take Exit 1 for CT-218W/Putnam HWY, and follow CT-218W.  Take the 1st left onto CT-187 S/Blue Hills Ave. and follow for .8 mi.  Turn left onto Lyme St., school is on the right.

BOLTON CENTER SCHOOL:  Notch Rd., Bolton, CT (643-2411):  Go to Rte. 44 in Bolton.  Go through Bolton Notch toward Manchester, bearing right and staying on Rte 44 past the old drive-in theater.  Turn left at the intersection of Rtes 44 & 85 onto Rte 85 (Bolton Center Rd).  Go up the hill and over I-384 staying on Bolton Center Rd for about 2 miles.  You will see a fire station on the left.  Turn left onto Notch Rd.  Proceed about ˝ mile.  The school will be on the right.

COVENTRY (742-7334): Follow Rte. 31 through Coventry. School is on the left between the police station and Coventry High School.

EAST WINDSOR (623-4488): (Games at Intermediate School) Take Route 140 in Ellington to light that leads to 140 intersection. Take left at light, continuing on Rte. 140. At next light, turn left onto Main Street (Rte. 191 So.). School is about ˝ mile on the right at the blinking light.

GRANBY MEMORIAL MIDDLE SCHOOL:  Take Rte. 84 W to Exit 61, I-291 W.  Take Exit 2B to merge onto I-91N towards Springfield.  Take Exit 40 to merge onto CT-20W/CT-401 toward Bradley Airport.  Exit onto CT-20W/Rainbow Rd, toward East Granby/Granby, and continue to follow CT-20 for 6 miles.  Turn right onto Salmon Brook St., follow for .4 mi., turn left onto Crest Road, school will be on the left.

ILLING MIDDLE SCHOOL:  229 E. Middle Turnpike, Manchester, CT (647-3400):  Illing is next to the police station and near Manchester HS.

JFK MIDDLE SCHOOL:  155 Raffia Rd., Enfield, CT (763-8863):  Take 291 N to I-91 N to exit 46 (US-5/King St).  Take a left at the end of the ramp onto King St.  Take an immediate left at the light onto Weymeth Rd. (about 2 miles).  Take a left onto Simon Rd.  At the 4 corners stop sign, go straight.  JFK Middle School is on the right.  The fields are in back.

Or back roads from Tolland to 190W—follow through Somers into Enfield.  Take a left onto South Rd., then a left onto Raffia Rd.  JFK is 1.5 miles on the left.

Or back roads from Tolland to 140 W. through Ellington and E. Windsor.  Proceed straight through the traffic light where 191 S. turns left to go to EWMS.  Around the next corner, you will pass Ravey’s Greenhouses on the left.  Take your next right onto Yosky Rd.  Proceed straight through 4 stop signs, and JFK will be on the right.

LEBANON MIDDLE SCHOOL:  891 Exeter Rd., Lebanon, CT (860-642-4702):  Take Rte. 195 S. to Rte. 32.  Turn right onto Rte. 32 toward Willimantic.  In Willimantic at the stop light with Dunkin Donuts and Bennie’s on the left, turn Right onto Bridge St. which is still Rte. 32.  Follow around the hairpin turn.  At the 4 way stop, go Straight onto Rte. 289.  Follow Rte. 289 approximately 5 miles to the stop sign.  Bear left onto Rte. 87.  Go .8  of a mile to stop sign.  Turn right onto Rte. 207.  School is 1.25 miles on the left.

MANSFIELD MIDDLE SCHOOL (429-9341): Take Rte. 195 through the UCONN campus. At Spring Hill Rd. (past the white church) take a right. Follow to the end. Take a left. School is on the left.

SOMERS MILDRED B. AVERY MIDDLE SCHOOL:  Take Rte. 30 N/Crystal Lake Rd. Bear left onto Burbank Rd.  Continue on Gulf Rd for 2.7 miles.  At Rte. 190, turn left (Main Street in Somers).  Turn Left at 9th District Rd., school is on the left.

STAFFORD MIDDLE SCHOOL (684-2785): Go to Rte. 190 in Stafford. Take Rte. 32 North and follow signs to Stafford High and Middle Schools.

SUFFIELD MIDDLE SCHOOL:  Take Route 84 W to Route 291W.  Merge onto Route 91N.  Take Exit 40 onto CT-20W.  Take the CT-75 exit bearing right toward Poquonock/Suffield.  Turn slightly left onto South Main Street.  Turn left at CT-168W/Mountain Road.  Suffield Middle School will be on the right.

TIMOTHY EDWARDS MIDDLE SCHOOL:  Take Rte. 84 W  to Exit 64.  Turn left at Kelly Rd., then turn right at Avery St.  Take the first right onto Gray Rd.  Take the 3rd left onto Arnold Way to TEMS.

VERNON CENTER MIDDLE SCHOOL:  Follow Rte. 30 into Vernon.  VCMS is on the right.

WINDHAM MIDDLE SCHOOL:  Take Rte. 195 South towards Storrs.  Take a right onto Rte. 32, and follow for about 9 miles.  Take a left onto Roanoak Ave., then the 2nd right onto Quarry Street.  Windham Middle School is on the left.

WINDSOR LOCKS MIDDLE SCHOOL (292-5012): Take Route 140 through Ellington to the light that leads to the Rte. 140 intersection. Take a left at the light that leads onto Rte. 140 and follow it all the way to the center of Windsor Locks. At the light over the bridge, go straight. Follow this road to the end. Take a left. The school is at the bottom of the hill on the left hand side. OR  Take I 84 west to 291 north to I 91 north.  Take exit 42 to rte 159 and Windsor Locks.  Turn left at the end of the exit to 159.  Go straight through the 1st light onto 159 north.  Turn left onto Elm St.  Turn right onto Center St.  School is on the right.  Playing fields are out behind the school.
